Mostly, I think, because of the contrast between Karkat's alertness and John's; that he doesn't think John is ever alert except when fighting says a lot about Alternian expectations of public behaviour (and, naturally, threat level). And then John almost falls asleep?! His shock and disturbance are so palpable there, mmm.
